Overview

The RN Case Manager performs comprehensive patient assessments and implements individualized plans of care for those with life-limiting illnesses. They also collaborate with interdisciplinary teams and physicians to ensure coordinated, compassionate care and regulatory compliance.

Overview

The Hospice Aide will provide direct patient care to patients under the direction of the RN and according to the established Plan of Care, including assisting with safe transfers and ambulation. Responsibilities also include appropriately reporting changes, documenting care provided, practicing infection control, and ensuring compliance with all relevant laws and regulations.

The Nurse Practitioner will consult with patients and families regarding disease processes and end-of-life care while assisting the interdisciplinary team in developing patient plans. They are also responsible for collaborating with physicians, monitoring patient needs, and ensuring compliance with healthcare regulations.
