About the role

The Advanced Practitioner will provide outpatient medical care to students in the School Link program, averaging 14-20 patients per school day. Overview UnityPoint Clinic is actively recruiting for an Advanced Practitioner to join our School Link program in Rock Island, IL UnityPoint Clinic is a physician-led, nonprofit health care organization offering comprehensive services in primary care, walk-in care and specialty care. The clinical teams at the area’s clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being. Opportunity to practice outpatient medicine at the School Link program supporting Rock Island Public Schools 1.0 FTE, 5 days a week Cohesive group of colleagues and supportive staff allowing for autonomy Experienced staff, both clinical and non-clinical, who know the patients and the community Electronic Medical Record: Athena Health and Phreesia

Responsibilities

Average of 14-20 patients per school day; no nights or weekends Potential for rotating call responsibilities/ directions to patients needs via triage Ability to work as a team member to improve the health of the people and communities we serve Evaluates, interprets and coordinates information to make decisions and recommendations regarding patient care Patient-focused, friendly and willing to collaborate in a team environment Excellent communication (written and verbal), analytical and problem-solving skills

Qualifications

NP Board Certified Ability to obtain and hold an unrestricted license to practice in the state of Illinois Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioner field of practice We offer Salary based position of approximately $110,000 – $115,000 A competitive Total Rewards program offers benefits that align with your needs and priorities, no matter what stage of life you are in.
