Key Responsibilities Clinical Care & Supervision Direct and monitor the daily functions of nursing assistants Ensure quality resident care in alignment with individualized care plans Admit, transfer, and discharge residents as needed Administer medications and treatments per physician orders Monitor, document, and report changes in resident conditions Documentation & Reporting Accurately complete and maintain resident medical records Record physician orders, chart treatments, and update care plans Prepare required forms, evaluation summaries, and progress notes Leadership & Communication Provide guidance and oversight to CNA staff Conduct routine rounds to ensure care quality and staff compliance Communicate effectively with residents, families, physicians, and interdisciplinary teams Participate in staff meetings, in-services, and ongoing training Compliance & Safety Follow infection control, safety standards, and facility protocols Participate in audits, surveys, and quality assurance activities Maintain confidentiality of resident and facility information Qualifications Current, valid California LVN license CPR certification required Graduate of an accredited LVN program Knowledge of Medicare/Medi-Cal regulations preferred Experience with PointClickCare (PCC) preferred Strong written and verbal communication skills Ability to multitask and work independently Must be able to lift 25 lbs and perform essential physical job duties Why Join Visalia Post Acute?
