The role involves implementing Person Centered Support Plans and assisting individuals with daily living skills such as meal preparation, hygiene, and housekeeping in residential or community settings. Employees must also administer medication, complete required documentation, and report any incidents affecting the individual's well-being.
Prior direct care experience is preferred, though not required, and a High School diploma or equivalent is preferred. Candidates must be able to perform physical tasks regularly, including lifting up to 50 pounds, standing for periods, bending, and providing physical interventions.
Implement Person Centered Support Plans for assigned individuals. Assist with daily living skills including but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities. Administer medication and treatment(s). Complete documentation as required through the general course of duties. Report to the appropriate Level 2 of Level 3, Program Manager, Case manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual. Attend all meetings as assigned and required. If approved as a driver of agency vehicles, employee may transport individuals. Other duties as assigned.
