The Occupational Therapist conducts therapy evaluations, develops treatment plans, and manages discharge planning for home-based patients while communicating closely with the multidisciplinary Hospice team. Responsibilities also include educating patients/families, supervising home health aides, and ensuring quality patient care according to established policies.
Requirements summary
Candidates must possess a Bachelor's or Master's Degree in Occupational Therapy (or a Ph.D.) and have at least one year of experience in acute care, rehabilitation, or home care, with proficiency in English. A California Occupational Therapist License is required upon hire.

Provide occupational therapy sessions centered on patient functionality and goals. Document therapy progress with the support of AI technology, reducing after-hours workload. Deliver care with full attention and connection to patients. Patient-Centered Care: Spend more time providing hands-on, compassionate care to patients. Reduced Documentation Burden: Use AI-supported tools to streamline clinical notes and regain personal time. Work-Life Balance: Cut down on after-hours documentation, allowing more time for family and personal priorities. Education and/or Experience Bachelor of Science in Occupational Therapy required. Two (2) years of experience as a Occupational Therapist in an acute care or rehabilitation setting. Certifications, Licenses, and Registrations Active and unencumbered Occupational Therapist license in state(s) of employment required. Required within 30 days of hire - National Provider BLS
