Job Requirements A Day in the Life of a Social Worker MSW

Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Conducts social work visits and care coordination; documents in medical record as appropriate.

Performs additional follow-up visits as indicated or determined by clinical professionals involved with care; documents as appropriate.

Establishes and maintains a supportive professional rapport with patient, family, physicians and other members of the interdisciplinary team.

Provides patient and family education on community resources and reinforces the plan of care with education, counseling and family/patient conferences as needed / ordered by physician.

Evaluate patient and family response to treatment/plan of care and education.

Helps patients understand their diagnosis and make the necessary adjustments to their lifestyle, housing, or healthcare.

Assess clients’ needs, situations, strengths, and support networks to determine their goals.

Desired Skills: Excellent oral/written communication skills necessary to positively interact with patients, patient’s families, visitors, physicians and other staff Ability to function independently and as a team member Basic computer skills Ability to assess, evaluate, explain, teach, problem solve using critical thinking; and perform calculations Ability to work rotating weekends Here is What You Need: Licensed by Wyoming Mental Health Professions Board Cheyenne Regional AHA RQI within 14 calendar days of start date Nice to Have: Two years of experience as a social worker Experience in grief and bereavement About Cheyenne Regional: Cheyenne Regional Medical Center was founded in 1867 as a tent hospital by the Union Pacific Railroad to treat workers injured while building the transcontinental railroad.

Requirements

Candidates must be licensed by the Wyoming Mental Health Professions Board and complete the Cheyenne Regional AHA RQI within 14 days of starting. Desired qualifications include two years of social worker experience and background in grief and bereavement.
