About the role

The Care Coordinator initiates contact with referred adult members (18+) of Comprehensive Community Services, building rapport using a strength-based, trauma-informed, and culturally sensitive approach while facilitating all aspects of service planning as outlined in DHS 36. Responsibilities also include ensuring coordinated, monitored service delivery designed to support member independence, assisting with self-advocacy, and coordinating crisis services.

We are searching for a CCS Care Coordinator!

Some of the ways we assist our care coordinators with maintaining a work/life balance and financial wellness is by encouraging a hybrid worksite schedule and paying quarterly productivity bonuses. Care Coordinators are able to work from home, in the office, and out in the community!

Requirements

Applicants must have a valid Wisconsin driver's license, vehicle and auto insurance, Bachelor's degree (B. A. / B. S.) from a four-year college or university; and one-year related experience and/or training; or equivalent combination of education and experience.

Job Duties Include

  • Initiates contact with the referred adult members, ages 18+, of the Comprehensive Community Services.
  • Builds rapport using a strength-based, trauma-informed, recovery-oriented, and culturally sensitive approach.
  • Facilitates all aspects of the Comprehensive Community Services service planning as outlined in DHS 36.
  • Ensures that the member receives culturally, linguistically, and age-appropriate assessment services appropriately and timely.
  • Ensures that each member's service plan and service delivery is coordinated, monitored, and designed to be as supportive as possible towards the member's independence.
  • Assists the member in self-advocacy and helps the member obtain other necessary services and support.
  • Completes and submits all required or assigned documentation by due dates.
  • Attends all supervision, meetings, training, in-services, and events as directed.
  • Maintains the members' records and ensures that records comply with state and county expectations.
  • Submits all service authorization requests to the mental health service director for review and submission.
  • Completes at least 8 hours of ongoing training annually.
  • Coordinates crisis services for the member to be provided by DHS 34 certified programs.
  • Participates in quality improvement projects.
