The Targeted Case Manager will develop and monitor Individual Recovery Plans (IRPs), provide strengths-based supportive contact, and develop relationships with community partners. Duties also include providing crisis case management, monitoring medication efficacy, assisting with housing, and advocating for consumers in the community.
Applicants must possess a Bachelor's degree (B.A. / B.S.) from a four-year college or university, along with one year of related experience or training. A valid Wisconsin driver's license, vehicle, and auto insurance are mandatory requirements for this role.

Applicants must have a valid Wisconsin driver's license, vehicle and auto insurance, Bachelor's degree (B. A. / B. S.) from a four-year college or university; and one-year related experience and/or training; or equivalent combination of education and experience.
