Discharge Planning Assistant

• Wonderful opportunity to learn more about the behavioral health care field, clinical work and the continuation of care and to obtain training.
• Assist in discharge planning on units as assigned by the Administrative Director of Social Work.
• Work with the treatment team and Program Director on the unit to assist in completion of applications and other discharge plans for the patients.
• Attend morning report and treatment planning.
• Gather necessary documents to complete applications and make appointments, make follow up calls and provide necessary information to outpatient providers.
• 2 years of college required, Bachelor’s preferred, hospital related experience is a plus.
• M-F 8:30 to 4:30.

$18.50 starting salary scale. Additional for college degree.
