The Wellness Coach will ensure seamless integration of care by accepting hand-offs from providers, engaging patients in goal setting, and delivering evidence-based health education both individually and in groups. This role also involves case management, tracking progress, coordinating referrals to community resources, and scheduling appointments across various service lines.
A Bachelor's Degree in Social Work, Health, or a Human Services field is preferred for this role. Candidates must have a minimum of three years of experience meeting the listed qualifications, and successful completion of a Level 2 background screening is required.
