Home Care Physical Therapists utilize advanced technology and clinical expertise to deliver exceptional, one-on-one care and monitoring to patients within their homes. This involves working collaboratively within a multidisciplinary team, including physicians, nurses, social workers, and home health aides, to provide compassionate community care.
Candidates must possess a Bachelor's degree with progressive experience or a Master's degree, along with a current Physical Therapist license in Illinois. A valid driver's license and reliable transportation are mandatory, and current working knowledge of conditions of participation and prior home healthcare experience are preferred.
