The social worker plays a vital role in supporting the emotional, psychosocial, and discharge planning needs of sub-acute residents. Conducting comprehensive psychosocial assessments upon admission and updating them regularly. The social worker actively participates in interdisciplinary care plan meetings and coordinates services for safe and timely discharges. They provide counseling to residents and families, advocate for residents’ rights, and promote quality of life. Acting as a liaison, the social worker fosters communication among residents, families, staff, and outside providers. This role involves conflict resolution, maintaining compliant documentation, and helping residents adjust to facility life. The social worker compiles social histories, prepares assessments and care plans, and revisits them quarterly or as needed. They assist residents and families in understanding care goals and limitations, and educate staff on the psychosocial aspects impacting residents. Address and resolve concerns using effective conflict resolution strategies Maintain accurate and timely documentation in compliance with regulations Collaborate with clinical staff, case managers, and external providers

Requirements

Minimum of a Bachelor’s degree in Social Work or a related field, Master’s degree in Social Work (MSW) preferred. LMSW licensure, preferred Experience in sub-acute or skilled nursing settings, strongly preferred. Knowledge of Medicare/Medicaid guidelines Strong organizational, communication, and interpersonal skills Minimum of two years of experience in the social services field, preferably in a nursing facility, hospital or related agency. Pleasant personality and ability to work with residents, families, and staff from various departments. Proficient in Microsoft Office. Ability to work well under pressure and be flexible.
