The LPN is responsible for managing individualized patient care through assessment, triage, and the administration of treatments and medications. They collaborate with a multi-disciplinary team to promote patient health and manage daily clinical workflows including referrals and lab results.
Candidates must have a nursing diploma, a current South Carolina nursing license, and CPR/BLS certification. At least two years of experience in a primary or specialty care medical practice is required.

The Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient’s health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support.

Adequate to perform job duties in person and over the telephone.
Must be able to communicate clearly to patients in person and over the telephone.
Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens.
Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
