This full-time, permanent role requires providing direct support and case management services to assigned clients. The Social Worker will also be responsible for monitoring and improving the patient experience through focus on quality, timeliness, and continuity of care.
Candidates must be a Licensed Social Worker, with LICSW candidates being highly favored for this position. Prior background in a hospital setting is preferred but not a mandatory requirement.
