The Case Manager assists in implementing social program needs, focusing on resolving barriers to service engagement and ensuring linkage to medical and behavioral health care to increase client self-management. Duties include assessing eligibility for grant programs, developing treatment plans, providing advocacy and linkage to care, and maintaining client records in compliance with regulations.
Candidates must possess a Bachelor's degree, preferably in a healthcare or social sciences field, along with two years of case management experience, including collaboration with interdisciplinary healthcare teams. Reliable transportation and flexible scheduling, including weekends, are required for this role.
The Case Manager for ID assists in the full implementation of social program needs with a special focus on resolving barriers to engagement in services, ensuring linkage to medical and behavioral health care, with overall goal of increasing client’s ability to self-manage while facilitating integration of care for those individuals and families working within a multiple services systems. The ID Case Manager will assess all new and existing clients for grant program eligibility, answer and respond to inquiry calls, and provide program information and referrals for new and existing clients.
Work is normally performed indoors, in a clinic, office, or mobile health unit with varying temperatures.
Occasional work at outdoor events with varying temperatures as necessary.
