The Community Care Worker I determines eligibility for Community Care Services by conducting home visits and interviews, documenting information, and developing service plans. They also monitor services to ensure they meet individual needs and provide referrals to other programs.
Candidates must have a current valid driver's license and be willing to travel at least 80% of the time. A high school diploma or equivalent is required, along with experience using email and automated applications.

Current Valid Driver’s License.
Candidate must have reliable transportation. Must be able to attend training which may require overnight stay.
This position is included in a career track series and will be filled at the Worker I level. Applicants with previous employment with CCSE will be reviewed by program management to determine appropriate entry level.
