The Registered Nurse develops and implements an appropriate Plan of Care based on thorough assessment, documenting ongoing assessment, treatment, and patient progress toward goals. This role also involves communicating with providers, coordinating the POC with the Interdisciplinary Team, and supervising Licensed Practical Nurses and Licensed Nursing Assistants.
Applicants must be graduates of an accredited school of Nursing and possess a current New Hampshire nursing license or a compact state license. A minimum of one year of recent general medical/surgical or skilled/acute care nursing experience, along with a valid driver's license and reliable transportation, is required.

Position Overview Reporting to the Home Care Director/Manager, the Granite VNA Registered Nurse delivers age appropriate care to the patient in the home setting utilizing standard nursing best practice: assessment, planning, intervention, implementation and evaluation within scope of practice defined by the NH Board of Nursing. RNs will effectively interact with patients, care givers and other healthcare team members while maintaining all standards of professional nursing. RNs will supervise Licensed Practical Nurses and Licensed Nursing Assistants assigned to patients.
Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC) One year of recent general medical/surgical or skilled/acute care nursing experience Valid driver's license, reliable transportation and an acceptable driving record
