The Nurse Practitioner will perform primary care services in home settings, including patient screenings, assessments, and documentation. They will collaborate with other clinicians to manage patient health and engage patients in the WellBe program.
Candidates must have a Master's Degree in Nursing with a focus on Nurse Practitioner and an active Nurse Practitioner license in Pennsylvania. Additional requirements include board certification, BLS certification, and a valid driver's license.
Your Impact: Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients. Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions. Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital. Job Requirements What You Bring: Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family. Pennsylvania Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP). Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
