The Hospice Nurse Practitioner will conduct essential Face-to-Face encounters for hospice eligibility certification, gathering clinical information and performing detailed assessments. This involves documenting findings, adhering to LCD criteria, and collaborating with physicians and the interdisciplinary team to ensure quality and compliance.
Candidates must possess a Master’s or higher degree in Nursing, completion of an accredited graduate-level NP program, and current state licensure/certification as an Advanced Practice Nurse. Strong skills in patient assessment, understanding of hospice principles, and familiarity with regulatory guidelines are required.
