The Advanced Practice Pharmacist manages complex and chronic diseases under a Collaborative Practice Agreement, authorized to prescribe, adjust, and discontinue medications to optimize therapeutic outcomes. Core duties involve clinical assessments, ordering/interpreting labs, monitoring patient responses, and collaborating with the care team to ensure safe and cost-effective patient management.
Candidates must possess a Doctor of Pharmacy (PharmD) or Bachelor of Science (BSPharm) and meet California APP requirements, typically involving board certification or residency/extensive clinical experience. Required licensure includes Registered Pharmacist (RPH) in California, BLS, and an APh license within six months of hire.
The Advanced Practice Pharmacist (APP) in the Pharmacotherapy Clinic at John Muir Health plays a vital role in the comprehensive management of patients with a variety of complex and chronic disease states. Practicing under a Collaborative Practice Agreement (CPA) in accordance with California State Board of Pharmacy regulations, the APP is authorized to prescribe, initiate, adjust, and discontinue medications to optimize therapeutic outcomes across a range of specialties. Core responsibilities include assessing medication regimens for safety, efficacy, and adherence; streamlining therapies when appropriate; mitigating drug interactions; and educating patients and caregivers to support long-term treatment success. Pharmacists will perform clinical assessments, order and interpret laboratory tests, and monitor patient responses to therapy to ensure safe, timely, and cost-effective care. The APP collaborates closely with physicians and the care team to ensure continuity of care and achievement of clinical goals. Strong documentation and communication skills are essential in both patient-facing and interdisciplinary team settings. In addition to direct patient care, the APP contributes to policy development, quality improvement, and staff education. Responsibilities may also include supervising technicians, supporting regulatory compliance initiatives, and assuming leadership duties in the absence of pharmacy management. Success in this role requires clinical expertise, adaptability in a high-acuity outpatient environment, and a proactive, collaborative approach to patient care.
