Responsibilities

ESSENTIAL FUNCTIONS: Conduct social history assessment of the patient through observation and patient/family interview, obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan. Coordinate and plan programs and activities to meet patient’s social and emotional needs. Promote recovery, symptom reduction and the achievement of the highest level of social functioning. Provides crisis management and family sessions to assist the family in understanding the implications and complexities of a life situation and its impact on lifestyle. Assist in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient. Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan. Communicate or aid family members in understanding and supporting the patient. Manage a caseload and assume responsibility for patient chart documentation. Facilitate education, process groups, support groups and referrals as requested. Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority. OTHER FUNCTIONS: Perform other functions and tasks as assigned.

Qualifications

Education/Experience/Skill requirements:

  • Master's degree in Social Work from an accredited college or university or a new graduate of accredited Master's degree social work program with successful completion of social work field education, internship or similar practicum through master's degree program required. Two or more years’ experience working with patient population preferred. LICENSES/DESIGNATIONS/CERTIFICATIONS: State licensure, as required by the state in which the facility operates; which could include: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW) CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). Requirements

A Bachelor's degree in Social Work from an accredited college or university is required, including for new graduates. Candidates must be eligible for state licensure as an LSW, LAC, CAC, or RAC, and must possess or obtain CPR and de-escalation/restraint certification.
