The Behavioral Health Clinician will lead integrated behavioral health services for patients with mild to moderate mental health or substance use needs, coordinating care with psychiatrists. Responsibilities include conducting assessments, developing treatment plans, and providing brief interventions.
Candidates must have an active LICSW, LCSW, LMHC, or LPC license and at least 5 years of clinical experience in behavioral health care. Experience with high-risk populations and familiarity with trauma-informed care are also required.
Behavioral Health Clinicians are integral members of our Integrated Care Teams—alongside physicians, advanced practice clinicians, health advocates, patient navigators, social workers, nurses, psychiatrists, and pharmacists—delivering holistic, patient-centered care to individuals with complex medical, behavioral, and social needs.
As an Accompany Health Behavioral Health Clinician, you will primarily provide virtual care via video, phone, or text, with in-home or community visits as needed. You will lead integrated behavioral health services for patients with mild to moderate mental health or substance use needs under the Collaborative Care Model. In collaboration with psychiatrists, you will coordinate Advanced Behavioral Health care for patients with serious mental illness and/or substance use disorder, ensuring a behavioral health perspective informs all aspects of comprehensive care planning.
