About the Role

The Registered Nurse (Sub-Acute) plays a critical role in delivering high-quality nursing care to patients requiring specialized sub-acute medical attention.

This position focuses on managing complex patient needs, coordinating multidisciplinary care plans, and ensuring smooth transitions from acute care settings to sub-acute environments.

The nurse will monitor patient progress, administer treatments, and provide education to patients and their families to promote recovery and prevent complications.

Working primarily during the morning shift, the nurse will collaborate closely with physicians, therapists, and other healthcare professionals to optimize patient outcomes.

Ultimately, this role is essential in maintaining a safe, compassionate, and efficient care environment tailored to the unique needs of sub-acute patients.

Minimum Qualifications

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Completion of an accredited nursing program (Associate’s or Bachelor’s degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of 1-2 years of clinical nursing experience, preferably in sub-acute, rehabilitation, or long-term care settings.
  • Strong knowledge of nursing principles, patient care standards, and relevant healthcare regulations.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) degree.
  • Advanced certifications such as Certified Rehabilitation Registered Nurse (CRRN) or similar.
  • Experience with electronic health record (EHR) systems and clinical documentation software.
  • Familiarity with quality improvement processes and patient safety initiatives.
  • Excellent communication and interdisciplinary collaboration skills.

Responsibilities

  • Assess and monitor patients’ health status, including vital signs, symptoms, and response to treatments.
  • Administer medications, intravenous therapies, and other prescribed treatments accurately and safely.
  • Develop, implement, and update individualized care plans in collaboration with the healthcare team.
  • Provide patient and family education regarding care plans, medications, and post-discharge instructions.
  • Coordinate with physicians, therapists, and other healthcare professionals to ensure comprehensive care.
  • Document all nursing care, patient progress, and any changes in condition in compliance with regulatory standards.
  • Respond promptly to patient needs and emergencies during the AM shift, ensuring patient safety and comfort.
  • Participate in quality improvement initiatives and maintain compliance with facility policies and procedures.

Schedule

  • Fulltime - 4x2
  • NOC - 7:00
  • AM - 3:00 PM
