The Physical Therapist is responsible for evaluating residents for short-term skilled rehabilitation or long-term care due to chronic conditions. Key duties include providing quality physical therapy services, developing plans of care, supervising assistants, and completing necessary documentation per guidelines.
Candidates must possess a Master’s degree or higher from an accredited physical therapist program and have a passing score on the national physical therapy exam. A current, active Physical Therapist license in Oklahoma and the ability to meet documentation requirements are mandatory.
Provides quality physical therapy services and recognizes referrals when appropriate, in accordance with established standards of practice, company procedures, therapy department procedures, and productivity standards Evaluates and obtains data necessary to ensure the plan of care reflects the safety and overall well-being of the individual Provides ongoing supervision for Physical Therapy Assistants as agreed upon through board certified Form 5 to ensure justification for the need of skilled services and proper implementation of plan of care Educates/coordinates with the patient, family, outside contractors, and/or staff as needed regarding appropriate physical therapy techniques to allow carryover of therapeutic remediation and goals in daily activities Completes required daily treatment notes, progress notes, recertifications, and discharges to justify skilled necessity and to ensure best quality of care per Medicare guidelines QualificationsRequirements: Master’s degree or higher from a physical therapist accredited program Must have passing score on the national physical therapy exam Current, active licensure in the state of OK as a licensed Physical Therapist Uphold state required CEU’s in order to maintain good standing with the board Must be able to complete documentation efficiently to meet reimbursement and regulatory requirements Therapeutic experience in older adults across multiple diagnostic groups is preferred Valid driver’s license, and an automobile in good working condition with appropriate insurance coverage in accordance with state laws Respectful, open communication with other team members, clients, and families
