The Community Health Worker (SDOH) acts as a patient advocate and facilitator within the Patient Centered Medical Home, focusing on high-risk patients by providing health education, monitoring follow-up, and improving access to care. Key duties involve assessing social needs using the PRAPARE assessment, identifying needs, providing social referrals, and developing tailored care plans to decrease health disparities and improve outcomes.
Candidates must be familiar with common Windows-based software and demonstrate an ability to quickly learn new systems, alongside proficient computer skills including typing, internet navigation, and email. Required qualifications also include current OSHA training and maintaining BCLS certification biannually.

Waikiki Health is seeking a full-time Community Health Worker (SDOH).
Pay rate for this role is $22 per hour
Schedule: Monday – Friday, 40-hour work week
The Community Health Worker (SDOH) is responsible for improving outcomes for high-risk patients, who utilize the ED/Hospital regularly and/or that have been identified using our Social Determinants of Health (SDOH) screening tool, PRAPARE. This position will help achieve better outcomes by working with an interdisciplinary team. The Community Health Worker (CHW) will assist in providing services during clinic hours, participating in huddles, patient intakes, education, and counseling, within clinic protocols.
