This Case Manager provides ongoing community psychiatric support treatment services to clients with serious and/or persistent mental illness co-occurring with substance abuse disorder, assisting them in receiving wrap-around rehabilitation services. Responsibilities include collaborating on treatment plans, implementing programming, monitoring medications, assisting with daily living skills, and acting as an advocate for client needs and entitlements.
A High School diploma is required, though a Bachelor's degree is preferred; non-Bachelor's candidates must hold specific Ohio certifications. Candidates need a minimum of three years of experience working with severe mental illness or substance abuse, must possess a car and valid Ohio driver's license, and demonstrate strong organizational and teamwork abilities.
