Responsibilities

This position serves in a dual role, with approximately 50% of time dedicated to case management duties and 50% focused on client medical coordination and support. As part of the LSSSD team, the case manager supports and strengthens individuals and families by developing client-centered case plans, providing ongoing assessment, and assisting clients in accessing community resources to meet their needs.

Responsibilities

include maintaining accurate case documentation; building collaborative relationships with community partners and service providers; coordinating medical, dental, and vision appointments; ensuring annual exams are completed; supporting medication management processes in collaboration with nursing staff and pharmacy providers; obtaining medical records and treatment consents; reviewing Medication Administration Records (MARs) for accuracy; and transporting clients to appointments as needed.

This position participates in the on-call rotation system.

Qualifications

  • A bachelor’s degree in social work or a related field, is required.
  • Bi-lingual is a preferred but not required.
  • Must be med-aid trained, familiar with electronic Medication Administration Record (MAR).
  • Must be at least 21 years of age, is required.
  • Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
  • Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
