The role involves providing direct patient care, including personal care and companionship, as directed by the plan of care under the supervision of a Case Manager. Responsibilities also include performing light housekeeping and meal preparation tasks while maintaining positive professional relationships.
Candidates must possess a High School Equivalent/GED and maintain required continuing education for certification, with 1+ years of home care experience preferred. Required credentials include a Certified Nursing Assistant (CNA) or equivalent, a valid OH Driver's License with insurance, and obtaining Basic Life Support (BLS) certification within 30 days.

Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Part time 20 hours per week. 100%25 travel to the patients homes to provide care. The territories are Ashland County. Monday-Friday.
High School Equivalent / GED required and Completes required continuing education requirements to maintain certification required. 1+ years Home care experience preferred. Certified Nursing Assistant (CNA) or equivalent certification required and Basic Life Support (BLS) Required within 30 Days and OH Driver's License (Valid) with car insurance coverage required. Successful completion of agency written test and competency evaluation of skills in the field Required within 30 Days.
