$45 - $50 / HOUR
The RN Case Manager will conduct in-depth client assessments and utilize clinical expertise to create personalized care plans focused on helping clients live independently. Responsibilities also include achieving client goals, such as safe medication management, and collaborating with team members to build client support networks.
Candidates must be compassionate Registered Nurses who have graduated from an accredited nursing program and possess a current Illinois RN license. A minimum of one year of RN experience, current CPR certification, and proficiency in safety and infection control are also required.

You're a graduate of an accredited nursing program. You have a current Illinois Registered Nurse (RN) license. You've got a minimum of one year of experience as an RN. You're CPR-certified and a pro at safety and infection control.
