$200 / HOUR
include providing comprehensive initial and ongoing patient assessments, establishing and revising treatment plans in consultation with the physician, and teaching/supervising the patient and family in the physical therapy program. Therapists must also write and submit clinical progress notes within 72 hours of the visit.
Candidates must be graduates of an AMA/APTA approved Physical Therapy school and licensed or registered by the State, requiring a minimum of one year of institutional or home health experience. Applicants must also pass a criminal background check and hold a current CPR (BLS) certification.
$200 for each start of care visit!

How we support you professionally: This position is paid at a competitive per diem rate Company 401K with 4% company matching
Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests Teach, supervise, and counsel the family and patient in the physical therapy program Write clinical/progress notes on the date of the visit and incorporates same in the patient's record within 72 hours.
The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds
Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association Must be licensed or registered by the State Must have at least one year of institutional/home health experience Must have criminal background check Must have a current CPR (BLS) certification
