The Hospice Physical Therapist will plan, organize, and deliver therapy services focused on maintaining patient comfort, function, and quality of life, guiding patients and caregivers through supportive techniques and education at the end of life. Essential functions include providing physical therapy services per the physician's plan of care, assessing functional status with a hospice focus, and instructing caregivers on therapy techniques.
Candidates must possess a Bachelor's or Master's degree in physical therapy from an accredited program and be licensed to practice as a physical therapist in the state of practice. Preferred qualifications include two years of relevant experience, especially in home health or community-based settings, along with a current CPR certification and a licensed, insured automobile.
