The Nurse Practitioner will provide high-quality primary and palliative care to homebound patients in Salem County, NJ, which involves performing physical examinations, developing diagnoses, and creating treatment plans under physician supervision. Responsibilities also include effectively communicating with internal staff and consulting with patients, families, and caregivers during home visits.
Candidates must be Board Certified with a valid Nurse Practitioner or Physician Assistant certification in New Jersey, possessing strong clinical skills for accurate assessment and diagnosis. A minimum of 1-2 years of primary care experience with geriatric patients is required, and hospice experience is highly preferred.

We are looking for an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Salem County, NJ region. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.
• $$+ (depending on experience and other incentives)
