The Advanced Practice Clinician is responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues while identifying and addressing disease prevention and health promotion for the certified population. Position Highlights Patient centered team-based environment Provide care across the continuum, focusing on value-based care and supported by a robust team of specialists Behavioral health providers are integrated into the primary care teams Fewer patients per day, longer patient visits Flexible scheduling options State of the art/award winning EMR System Primary Responsibilities Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues.
Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population certified in the Practice while administering quality patient care.
Maintains accurate medical records and confidential medical records.
Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations.
Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.

Required qualifications include an unrestricted license in New York State as a Registered Nurse with the title of Nurse Practitioner/Advanced Practice Nurse OR a Licensed PA in NYS, along with an unrestricted DEA license or willingness to obtain one before starting. The role is for full-time employment with a salary range of $109,500 to $164,000 annually.
