About the role

The Advanced Practice Clinician is responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues while identifying and addressing disease prevention and health promotion for the certified population. This role involves maintaining accurate medical records, documenting all evaluations and treatments, and educating patients and families about preventative care and prescribed medical treatments.

Position Highlights Patient centered team-based environment Provide care across the continuum, focusing on value-based care and supported by a robust team of specialists Behavioral health providers are integrated into the primary care teams Fewer patients per day, longer patient visits Flexible scheduling options State of the art/award winning EMR System Primary Responsibilities Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues.

Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population certified in the Practice while administering quality patient care.

Maintains accurate medical records and confidential medical records.

Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations.

Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.

Required Qualifications

  • Unrestricted licensure in the state of (NY) at time of employment
  • Licensing: Registered Nurse with current unrestricted license to practice and use the title of Nurse Practitioner/Advanced Practice Nurse in NYS AND/OR Licensed PA in the state of NY Unrestricted DEA license (or willing to obtain prior to start) The salary range for this role is $109,500 to $164,000 annually based on full-time employment. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). Requirements
