This role involves providing intermittent skilled nursing services in home and community settings according to an established plan of care, under the direction of an RN. The nurse will also utilize teaching principles to support patient/family health restoration and prevent disability, while accurately recording daily progress in the EMR.
Candidates must possess a High School diploma or GED and have successfully completed a Connecticut state-approved LPN certificate program. Essential requirements include holding a current and valid LPN license in the state and maintaining a valid Connecticut driver's license with a good driving history.
