Job Summary: This position is accountable to provide diagnostic and treatment services to home health patients, including communication with physicians and other health care personnel to assist in providing quality patient care.
Duties and responsibilities are performed according to standards outlined in the competency-based evaluation tool.
Essential Functions & Responsibilities: Provides for the physical, mental and social well-being for individuals and families by assessing, planning, implementing and evaluating the results.
Provides appropriate and thorough teaching to patients and their caregivers.
Completes clinical records, schedules, and administrative tasks.
Serves as Case Manager to coordinate multidisciplinary services.
Provides interdisciplinary communication in order to share information and assist with patient management.
Participates in staff and administrative meetings.
Participates in performance improvement (PI) activities.
Other duties as assigned Qualifications: Graduate of an accredited occupational therapy program, required.
NBCOT examination passed, required.
Current Virginia State licensure, required.
AHA BLS Provider CPR required at hire Professional competency as a general practitioner as defined by the American Occupational Therapy Association, required.
Minimum of one year Occupational Therapy healthcare experience, required.
