The role involves assisting in the continuity of care for diabetic patients by collecting necessary history, performing physical exams, and documenting encounters with clear assessments and plans. Responsibilities also include coordinating all necessary services and referrals for effective diabetes management, such as patient education and diagnostic procedures.
Required qualifications include being a Nurse Practitioner (NP) with a Master's Degree in Nursing or a Physician Associate (PA) with current national certification, along with holding current Oregon NP or PA licenses and Prescriptive Authority/DEA License. Prior ambulatory and inpatient experience caring for diabetic patients is preferred.
