The Community Health Worker - Patient Navigator will function as part of the patient care team by executing clinic-based programs focused on addressing patient needs related to social determinants of health and conducting patient in-reach initiatives. This role involves learning about local community resources and culture to better understand patient needs and positively influence overall patient experience and health outcomes.
Candidates must possess an Associate's degree, or have experience in a health care setting that can be substituted year-for-year for the education requirement. A minimum of one year of experience in a health care setting, community health education, or an outreach position is required, along with fluency in English.

This role will learn and obtain knowledge around the resources and culture of the local community to better understand the needs of patients and have a positive impact on the overall patient experience and health outcomes.
