Sylva Valley Health & Rehab is hiring a Speech-Language Pathologist (SLP) to provide high-quality therapy services in a skilled nursing setting. This role is ideal for an SLP who enjoys functional, resident-centered care and strong collaboration with nursing, dietary, and the interdisciplinary team.
Competitive pay (based on experience) Flexible schedule options (discussed during interview) Supportive team culture and collaborative rehab environment Vacation time + Sick time (per eligibility) Key responsibilities Evaluate and treat residents for swallowing (dysphagia), communication, cognition, and voice needs Develop and implement individualized plan of care with functional goals Provide caregiver/staff education on swallowing safety, communication strategies, and recommended precautions Collaborate with nursing, dietary, and providers on diet recommendations and changes in condition Document evaluations, progress notes, and outcomes to support skilled coverage and care needs Participate in care conferences/IDT and discharge planning as needed
Current Speech-Language Pathologist license (state-licensed) Graduate of an accredited SLP program Strong documentation and communication skills SNF/rehab experience preferred (new grads considered with strong clinical skills)
