The primary responsibilities involve coordinating and accurately completing MDS assessments on time, managing ARD scheduling, and ensuring documentation supports skilled services and reimbursement under PDPM/PPS guidelines. This role also requires supporting case management needs, including clinical updates and payer communication, while collaborating with the interdisciplinary team.
Candidates must be an active Registered Nurse (RN) with required experience in MDS within a skilled nursing facility and proficiency in PointClickCare software. Strong organizational, communication, time management skills, and knowledge of MDS processes, care planning, and skilled documentation are essential.
Sylva Valley Health & Rehab is hiring an experienced RN MDS Coordinator to lead accurate assessments, strong clinical documentation, and coordinated care in our skilled nursing setting. This role is ideal for a nurse who is detail-oriented, deadline-driven, and confident managing MDS timelines while collaborating with the interdisciplinary team.
Competitive pay (based on experience) Consistent weekday-focused schedule (discussed during interview) Supportive leadership and collaborative IDT culture Vacation time + Sick time (per eligibility) Key responsibilities Coordinate and complete MDS assessments accurately and on time (OBRA/PPS as applicable) Manage ARD scheduling, tracking, and compliance timelines Ensure documentation supports skilled services and reimbursement (PDPM/PPS focus) Participate in/lead IDT and care plan coordination Support case management needs: clinical updates, payer communication, continued stay reviews as needed Collaborate with therapy, nursing, providers, admissions, and leadership to ensure appropriate level of care and accurate documentation Maintain compliance with facility policies and applicable regulations Required qualifications Active Registered Nurse (RN) license MDS experience required in a skilled nursing facility PointClickCare experience required Strong knowledge of MDS processes, care planning, and skilled documentation Excellent organization, communication, and time management skills Preferred (nice to have) Experience with PDPM/Medicare skilled documentation and managed care reviews MDS certification
