As a Physical Therapist on our team, you will be the lead architect of resident mobility and recovery: Evaluation & Treatment: Conduct comprehensive initial assessments and develop individualized, evidence-based treatment plans for residents. Functional Restoration: Provide high-quality therapy focusing on gait training, balance, strength, and fall prevention. Interdisciplinary Collaboration: Work closely with our Nursing leadership (DON/ADON), Occupational Therapists, and Speech Therapists to provide holistic, person-centered care.
Maintain timely and accurate clinical records using Matrix, ensuring compliance with Medicare/Medicaid and state guidelines.
Provide training to residents, families, and restorative nursing staff on safe transfer techniques and equipment use. Qualifications & Requirements Must be a graduate of an accredited Physical Therapy program. Must have one year experience as an Physical Therapist (geriatric rehabilitation setting preferred) Must have current license to practice as a Physical Therapist in facility-specific state Must have current certification in Basic Life Support.
