This position will assist our patients by utilizing a strong team-based approach to care, with a high integration of APP and MDs for the clinic, and inpatient and dialysis units for all 3 phases of nephrology care.
· Desired skill set: Experience in complex chronic disease management
up to 0.4 FTE per pay period
· 5% clinic work
· 5% hospital work
· 90% dialysis rounding
· Ideal candidate does not need to live in the Duluth area; candidate could live in areas near Virginia, MN or Grand Rapids, MN as there is travel involved with this position
· Schedule: Variable start and end times due to travel
12 (office), 12-15 (dialysis). Adult patients only (18+)
