The Case Management Assistant will coordinate and facilitate Team Conference meetings, manage essential documentation like referrals and admissions, and effectively communicate daily assignment updates. Responsibilities also include completing pre- and post-discharge activities to prepare patients, families, and clinicians.
A high school diploma or equivalent is preferred, along with proficiency in Microsoft Office products and strong, accurate typing skills. Preferred qualifications include knowledge of medical terminology and experience with insurance/payor systems.
