The Case Management Assistant will coordinate and facilitate Team Conference meetings, manage essential documentation like referrals and admissions, and ensure effective communication of daily assignment updates. Responsibilities also include completing pre- and post-discharge activities to prepare patients, families, and clinicians.
The role requires a high school diploma or equivalent, proficiency in Microsoft Office products, and strong, accurate typing skills. Preferred qualifications include knowledge of medical terminology and experience with insurance/payor systems.
