The specialist provides integrated clinical pharmacy services for an assigned area, focusing on evaluating, monitoring, and modifying patient pharmacotherapy based on evidence-based medicine and formulary compliance. Responsibilities also include participating in the development and management of cost-effective therapies, educating patients and providers, and documenting interventions related to prescribing and safety.
A Bachelor's Degree in Pharmacy or Pharm.D. is required, along with successful completion of both ASHP-accredited PGY1 and PGY2 residency training programs or a fellowship. Applicants must obtain or be eligible for a Texas Pharmacist License within 90 days of employment and demonstrate strong knowledge of drugs, disease states, and excellent communication and computer skills.
