This role involves promoting optimal social functioning for participants in the Prisma Healthy Start program by providing direct and indirect services, and supervising paraprofessionals, including Resource Mothers, in developing and evaluating the Social Work program. Key duties include developing Service/Treatment Plans, recruiting participants, ensuring Medicaid compliance through documentation review, and providing crisis management and case direction.
Candidates must possess a Master’s degree in Social Work from an accredited school and have three (3) years of experience in a Social Work-related area, with medical healthcare experience being preferred. Required licensure includes LMSW or Licensed Practitioner of the Healing Arts (LPHA), along with a Heart Saver certification.
Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference Supervises paraprofessionals and develops Service Plans/Treatment Plans to ensure participants are receiving proper care and services. Recruits and admits potential participants from the community to participate in the Prisma Healthy Start Program. Reviews all visitation forms and documentation generated by paraprofessionals to ensure that services are being provided and meet Medicaid requirements for billing. Audits and monitors quality assurance activities, to include chart audits and supervisory visits. Provides crisis management with the paraprofessionals on a regular basis to monitor progress towards goals, to problem-solve, and to implement strategies to promote maternal and child health. Directs case management with the paraprofessionals on a regular basis to monitor progress towards goals, to problem-solve, and to implement strategies to promote maternal and child health. Coordinates care in collaboration with members of the multi-disciplinary team to facilitate effective and efficient use of community and financial resources utilized by the participants served by the Prisma Healthy Start program. Provides assessment and follow-up of psycho-social and financial needs of participants based on individualized service-plans and potential needs of the participants and their family unit. Works within an interdisciplinary team in evaluation of clinical data; interprets data and makes recommendations based on results. Educates community regarding prenatal healthcare, child healthcare, and parenting skills through meetings with formal and informal community groups. Performs other duties as assigned by supervisor. Supervisory/Management Responsibility This is a non-management job that will report to a supervisor, manager, director, or executive. Minimum Requirements Education - Master’s degree in Social Work from an accredited school required. Experience - Three (3) years of experience in a Social Work-related area.
