The Team Coordinator will provide essential administrative support to clinical leadership and care teams, focusing on maintaining organized patient records, office files, and managing supplies. Key duties include handling communications, tracking equipment orders, inputting data into electronic systems, and assisting with scheduling, admissions, and discharge processes.
Candidates must possess at least two years of administrative experience within healthcare, with a preference for experience in hospice or home health operations. The role requires strong organizational skills to manage behind-the-scenes operations that ensure smooth functioning of care teams and patient support.
