Key Responsibilities

  • RN: Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients.
  • Develops, discusses, and communicates a realistic problem list (plan of care) in collaboration with each patient/family/significant other to address all identified needs.
  • Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient.
  • Evaluates the identified problems, care provided and patient's responses are reassessed to meet patient and family needs to ensure overall quality of care delivered.
  • Maintains up-to-date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.
  • Maintains current knowledge/certification/licensure.
  • Pursues professional growth and development.
  • Functions as a role model for current and new staff.
  • Provides direct and indirect patient care duties under the guidance and direction of the physicians and/or nurse practitioners.
  • Measures vital signs, performs EKGs, venipuncture, capillary blood glucose testing.
  • Interviews patients/clients and/or family members to obtain accurate health information and ensures information is documented in the patients/clients chart.
  • Assists patients/clients or family members with completion of medical forms (as needed).
  • Administers injections and medications, dresses wounds and incisions, interprets physicians’/nurse practitioners’ instructions to patients, assists with emergency and minor surgery, and performs related tasks as directed and needed.
  • LPN: Utilize nursing knowledge and skills in the safe implantation of basic preventative, therapeutic, and nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse.
  • Assists in data collection on the EMR.
  • Carries out physician orders accurately, directly, and indirectly.
  • Administers medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
  • Maintain and coordinate communication among patient, staff, and physician as evidenced within medical record, client, and health care team feedback.
  • Report patient needs/programs and observations regarding patient’s condition to the physician.
  • Work collaboratively with other members of the healthcare team in coordination of the patient’s care as evidenced by other department feedback.
  • Participates in staff development programs.
  • Takes direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
  • MA: Record patients' medical history, vital statistics, or information such as test results in medical records.
  • Prepare treatment rooms for patient examinations, keeping the rooms neat and clean.
  • Interview patients to obtain medical information and measure their vital signs, weight, and height.
  • Authorize drug refills and provide prescription information to pharmacies.
  • Clean and sterilize instruments and dispose of contaminated supplies.
  • Prepare and administer medications as directed by a physician.
  • Show patients to examination rooms and prepare them for the physician.
  • Explain treatment procedures, medications, diets, or physicians' instructions to patients.
  • Help physicians examine and treat patients, handing them instruments or materials or performing such tasks as giving injections or removing sutures.
  • Collect blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing.

Requirements

RN: Required Qualifications: Graduation from RN program and current RN license. Basic Life Support (BLS) required, must obtain within first 90 days of employment.

Preferred Qualifications

Minimum of two years nursing experience in a Clinic setting. Community health nursing experience. ER or Urgent Care experience. LPN: Required Qualifications: Graduation from LPN program and current LPN license. Basic Life Support (BLS) required, must obtain within first 90 days of employment.

Preferred Qualifications

Minimum of one year nursing experience in a Clinic setting. Community health nursing experience. MA: Required Qualifications: 1-3 years relevant clinical experience or current valid medical assistant credentials. Basic Life Support (BLS) required, must obtain within first 90 days of employment.

Preferred Qualifications

Graduation from a medical assistant program.
