The specialist will support patients in improving health metrics like blood sugar, blood pressure, and lipids through developing and implementing comprehensive care plans covering diet, exercise, sleep, and stress management. They will collaborate with prescribing providers on medication and empower patients using motivational interviewing and Continuous Glucose Monitor (CGM) data.
Candidates must possess a Registered Dietitian (RD) or Registered Nurse (RN) credential along with the Certified Diabetes Care and Education Specialist (CDCES) certification. Required experience includes demonstrating success in leveraging interpersonal skills and CGM data to achieve better blood sugar control.

What you’ll do We’re looking for an experienced RD/CDCES or RN/CDCES – referred to as a Diabetes Care Specialist at Ryse – to support patients virtually and in-person at our current and future clinics. You’ll build deep relationships with our patients and support them on their journey to better health while working closely with a top-tier team. Day-to-day you will: Support patients in better health – measured by blood sugar, blood pressure, and lipids – at lower cost Develop and implement patient care plans across the domains of diet, exercise, sleep, and stress management Collaborate with prescribing providers on medication choices Empower patients to reach their health goals by using motivational interviewing techniques and continuous glucose monitor (CGM) data Coordinate care with Ryse prescribing providers, behavioral health specialists, exercise specialists, and health coaches Counsel patients and families through video calls, in person meetings, telephone and electronic messaging Other activities to support patients to better cardiometabolic health in the context of team-based care About You Registered Dietitian (RD) or Registered Nurse (RN) and Certified Diabetes Care and Education Specialist (CDCES) required Demonstrated track record of leveraging interpersonal skills and CGM data to help patients achieve better blood sugar control Self-motivated and adaptable to changing processes Ability to balance multiple competing priorities, and keeping track of the little details Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease We want you to know Ryse Health is an Equal Opportunity Employer.
