The role involves providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting, focusing on implementing care plans for comfortable and safe dying and self-determined life closure. Responsibilities also include acting as a Family Educator/Advocate, maintaining patient dignity, and completing all required clinical documentation following agency protocol and guidelines.
Candidates must be a Registered Nurse licensed in the state of employment and possess a minimum of 3 years of inpatient clinical experience, preferably in hospice. A Bachelor's Degree in Nursing is preferred, and applicants must have a valid driver's license and auto insurance, along with the capability to meet all physical demands of the role.

HOW YOU'LL WORK: You'll be responsible for providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting in a manner consistent with standards of practice. You will assist in determining the need to change the level of care based upon assessment and collaboration with the Attending Physician or Medical Director. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family. Family Educator/Advocate: Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs, provide appropriate support at the time of death, including disposition of medications, notification of funeral home, etc. according to agency policy.
Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
Provide referral sources with information on hospice programs and the coordination of patient care for optimal transition from the hospital to discharge destination in a hospice care setting wherever the patient calls home.
