The Physical Therapist is responsible for conducting comprehensive patient assessments in the home setting and completing all necessary skilled documentation using Homecare Homebase. This role involves evaluating patients, performing appropriate therapy interventions, developing comprehensive plans of care, and coordinating multi-disciplinary care.

The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes. We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier — protecting your time. What you will do: Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits. Complete comprehensive in-home assessments and deliver skilled PT visits. Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. Set measurable goals, track objective progress, and adjust treatment quickly when patients stall. Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. Manage treatment and outcomes directly, including PTA supervision where appropriate. Coach patients and caregivers with practical home programs that actually get done. Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
